Guild of Imagination: Table Time Participation Waiver and Release of Liability

This waiver and release of liability is entered into by the parent or legal guardian of any minor child participating in Guild of Imagination: Table Time, operated by Guild of Imagination LLC and located at Kilburn Mills, 127 W Rodney French Blvd, Unit S3-22, New Bedford, MA 02744.

By signing the form, you acknowledge and agree to the following terms:

1. Voluntary Participation and Program Description

I understand that my child’s participation in Guild of Imagination: Table Time is entirely voluntary. The program involves structured tabletop roleplaying activities, including Dungeons & Dragons gameplay, storytelling, character interaction, and light physical movement within a shared studio space.

I acknowledge that while the activities are creative and primarily seated, there may be occasional movement, prop usage, or interaction with other participants in a small-group setting.

2. Assumption of Risk and Release of Liability

I understand that any group activity carries inherent risks. I voluntarily assume full responsibility for any injury, illness, accident, loss, or damage that may arise from my child’s participation, including but not limited to minor physical incidents, allergic reactions, emotional discomfort, or other unexpected outcomes.

To the fullest extent permitted by law, I hereby release and hold harmless Guild of Imagination LLC, its owners, employees, contractors, agents, representatives, and affiliates from any and all claims, demands, causes of action, or liability arising from or in connection with my child’s participation in the program, regardless of the cause, including negligence.

3. Medical Authorization

In the event of an emergency, I authorize Guild of Imagination staff to administer first aid or obtain emergency medical treatment if necessary. I understand that all reasonable efforts will be made to contact me first. I accept full financial responsibility for any medical care required.

4. Behavior Expectations and Dismissal Policy

I acknowledge that Table Time is a respectful, inclusive space that values creativity, cooperation, and safety. I agree that my child will follow all program rules and instructions from Guild Masters and staff.

I understand that inappropriate, unsafe, or disruptive behavior may result in my child being temporarily or permanently removed from the program. In such cases, no refund will be issued.

5. Photo, Video, and Media Release

I grant permission to Guild of Imagination LLC to photograph, film, and record my child during program sessions, events, and activities. These photos, videos, or audio recordings may be used for promotional purposes, including:

- Social media platforms (Facebook, Instagram, YouTube, etc.)

- Company website and landing pages

- Marketing emails and printed promotional materials

- Press releases or media coverage

I understand that my child’s last name or other identifying information will not be used without separate written consent. I waive any rights to inspect or approve the materials or receive compensation for their use.

If I do not wish my child to be photographed or recorded, I agree to notify Guild of Imagination in writing prior to the first session. Reasonable efforts will be made to honor that request.

6. Program Terms and Refund Policy

I understand that the Adventure Pass is a low-cost introductory offer designed to help new families experience the program. This fee is non-refundable unless covered by the first-session satisfaction guarantee. I agree to the terms and conditions provided at the time of registration.

7. Acknowledgment of Understanding

By signing the form, I certify that I am the parent or legal guardian of the child(ren) participating. I have read and fully understood the contents of this waiver and release. I understand that I am giving up legal rights by submitting this form, and I do so voluntarily.
